Payment is due at the time of service. We accept cash, check, Visa and MasterCard. We will provide you with a copy of your bill for you to submit to your company for reimbursement.
Please Note: Returned checks and a balance over 30 days are subject to a re-billing charge. No show of appointments may also be subject to an additional charge.
If you are on THE PARKWAY PLAN – You will have no paperwork, co-payment or payment at the time of service. If you wish to enroll in the Parkway Plan, please contact our Patient Representative. Click Here For More Information About The Parkway Plan.REFERRALS:
Your insurance policy may have specific guidelines for obtaining a referral for outside tests, consultants or treatment of any kind. It is very important that you check FIRST with our office about referrals to avoid any misunderstanding or unnecessary bills. As medical providers we work for you, our relationship is with you, not your insurers. All charges are ultimately your responsibility from the date service was rendered.
Your insurance is a contract between you and the insurance company. We are not party to that contract.
Not all services are a covered benefit in all insurance contracts. Please be familiar with what services are covered and what are not to avoid any misunderstanding and unnecessary bills.
